In order to sustain the vision we must continually re-evaluate and renew the partnership to assure appropriate, meaningful and relevant supports are being offered. This is accomplished through team meetings that occur at various times throughout the year and at an annual review of services.
As we grow older our needs change. There are transitional stages that each of us experiences throughout our lives and this is no different for someone who experiences a disability. These stages include our grade school years, our teens and high school, graduating from high school, entering into the “real” world and growing old. Each of these transitions needs to be recognized and evaluated, as a change in services and supports will be necessary to make them smooth. This is a very important part of renewing the partnership and being able to sustain the vision for the person choosing Hope’s supports.
