Hope
Community Resources, Inc. (Hope) provides a multitude of services
for many individuals throughout Alaska experiencing a developmental
disability. Financing these services comes from a variety of sources
such as private payment, settlements through court, insurance
proceedings, and most commonly, state and federal dollars designed
to fund the multiple service needs of these individuals. It is
important to note that the State of Alaska determines which funding
source is used for government supported services for any individual
based on his or her disability and functioning level.
State
Grant
These are funds provided through a grant from the State of Alaska.
Funds are allocated to each individual authorized to receive state-funded
services. This funding is transferable to another agency should
an individual elect to change service providers. Generally, an
individual must be determined eligible for developmental disability
services and drawn from the wait list. A service plan is developed
with assistance from a Care Coordinator and submitted to the State
of Alaska for approval.

and Community Based Waivers

This program is funded through a combination of state and federal
funds. Requirements are more extensive that grant funding; however,
approval of waiver services also means approval for Medicaid.
There
are four waivers in Alaska, each carrying its own particular set
of requirements. The four waivers are listed below with a very
brief description of eligibility requirements.
1.
Mental Retardation/Developmental Disability (MRDD) - Individuals
who qualify for an MRDD waiver must carry a diagnosis of mental
retardation, cerebral palsy, autism, seizure disorder or other
related conditions. They must also meet an Intermediate Care
Facility/Mentally Retarded (ICF/MR) level of care. There are
no age restrictions.
2.
Children with Complex Medical Conditions (CCMC) - Designed to
serve children 0 - 21 years of age. A child must meet a nursing
facility level of care.
3.
An Adult with Physical Disabilities (APD) - Designed to serve
adults ages 21 - 64. To qualify for this waiver an individual
must meet a nursing facility level of care.
4.
Older Alaskans (OA) - Designed to serve individuals 65 years
of age and older. Individuals must meet a nursing facility level
of care.
