
Hope
Community Resources, Inc. (Hope) has an exceptional team of
Community Health Nurses who are skilled in working with children
experiencing developmental disabilities and complex medical
conditions. One of Hope's nurses is nationally certified in
Developmental Disability Nursing.
The
majority of families choosing Hope's support for their children
obtain medical services through State of Alaska waivers, specifically
"Children with Complex Medical Conditions" (CCMC)
waivers. This particular waiver allows for the provision of
services to individuals from birth through age 21 who have complex
medical or skilled nursing needs. Services are provided in the
child's home which gives the individual the opportunity to live
at home regardless of his or her medical complexity.
Hope's
CCMC nurses work with families while they go through the waiver
application and planning process. This process typically begins
by meeting with the individual and his or her family to discuss
and assess the individual's needs in conjunction with the family's
needs. As the plan develops, the CCMC nurse continues to build
a relationship with the individual and his or her family, bridging
the gap to resources in the community.
Hope's
CCMC nurses travel to remote areas around the state to provide
individualized supports where they are most needed, in the child's
home and community. Nurses are available to coordinate specialized
training for family and other caregives, identify ongoing needs
and take steps to resolve those needs. Health and safety are
the main focus of Hope's CCMC nurses in an ongoing effort to
provide the best quality of life for the individuals and their
families.

- CCMC needs assessments
- home school and hospital visits
- CPR and First Aid instruction
- preparation of individualized training materials
- individualized training related to specialized needs
- assistance with ongoing follow-up and medical support coordination
- liaison with medical supply vendors
