Hope
Community Resource, Inc.'s (Hope's) team of Community Health Nurses
(CHNs) specializing in Adult and Mental Health nursing has more
than 65 years of experience in the developmental disability field.
The nurses combine knowledge, training and professional experience
to assist people and their families with the creation of individualize
plans outlining strategies to meet health care needs. Two of Hope's
nurses are nationally certified in Developmental Disability Nursing
and another is certified in Gerontological Nursing.
Hope's
team of nurses provides support to adults experiencing developmental
disabilities and adults experiencing dual diagnosis of developmental
disability with mental illness in the following ways:
- Collaboration with other professionals to attain equitable
access to mental health care for individuals, families and communities.
- Encouraging maximum and appropriate use of community health
related care opportunities, especially when they increase a
person's positive interaction with his or her community.
- Assisting with the assessment of an individual's health and
psychosocial status and monitoring the management of the individual's
behavioral and health plans.
